Opportunity:
Occupational Therapist – Occupational Therapy
Job Details:
• Work Monday through Friday with shift times approximately 7:45 AM to 4:00 PM
• No weekend rotation, no on-call responsibilities, and no holiday expectations
• Home health position covering primarily Racine and Kenosha Counties in Wisconsin
• Expected caseload of 6 patients/points per day with evaluation visits counting as 1.5 points and standard visits counting as 1 point
• Treat all adult patients including geriatric populations with diagnoses such as post-stroke, post-joint replacement, weakness, deconditioning, COPD, and CHF
• Focus on ADLs and adaptive equipment as primary OT treatment and evaluation skills
• Collaborate with RN and PT interdisciplinary teams with a morning team huddle via phone each day
• Most patients are within 20-30 minutes of travel, with occasional patients up to approximately one hour away
• Minimum 3 years of OT experience required; SNF, home health, or acute care background accepted with home health experience preferred
• Bachelor's degree in occupational therapy accepted
• Required certifications and licensure: BLS and Wisconsin OT license
• Must have a valid driver's license, personal vehicle, and active auto insurance
• EMR system: EPIC experience preferred
• Dress code: Business casual or any color scrubs
• Parking at patient homes

This home health organization provides skilled therapeutic services across multiple counties in Wisconsin, delivering patient-centered care directly in the home setting. The team operates with a strong interdisciplinary model, pairing occupational therapists with registered nurses and physical therapists to ensure comprehensive care for adult and geriatric patients. Support is provided through an RN supervisor, PT lead, PT manager, and a collaborative team of occupational therapists, ensuring clinicians have the resources they need to succeed in an independent home health environment.
