Opportunity:
RN – Case Management
Job Details:
• Monday through Friday shifts from 8:00 AM to 4:30 PM; four 10-hour shifts may also be available
• No weekend rotation required
• No call responsibilities
• No holiday expectations
• 1:1 patient ratio
• Work with adults only, specifically end-of-life patients
• Provide end-of-life care and planning, wound assessments, care coordination, pain and symptom management, and triage
• Float anywhere within the serviced counties as needed
• Work alongside RNs, Resource Nurses, and Case Managers
• Utilize EPIC Remote Client EMR system (experience preferred) and Sapphire IV pumps
• Orientation of up to one week based on comfort level
• Scrub color: any color or business casual attire accepted
• Required certifications: BLS, valid driver's license and insurance
• Minimum 2 years of hospice experience required
• EPIC Remote Client experience preferred
• First-time travelers accepted with hospice and EPIC Remote Client experience

This home health facility services North Milwaukee and the surrounding counties of Washington, Ozaukee, Waukesha, and Jefferson. The organization specializes in hospice and end-of-life care, providing compassionate in-home services to adult patients. The facility operates with a dedicated team of RNs, Resource Nurses, and Case Managers to support staff and ensure quality patient care across the region.
