Opportunity:
RN – Home Health Nurse
Job Details:
• 13-week contract with 40 guaranteed hours per week
• 8-hour day shifts, approximately 8:00 AM to 4:30 PM with potential flexibility in start and end times based on census
• Every 5th weekend and holiday rotation required
• On-call required 2-3 times per month
• Primary location is Wausau Home Health with potential to float to Medford, Antigo, and WI Rapids on a limited basis
• Travel to patient homes required, averaging 25-60 miles per day depending on territory, patient census, and staff availability
• Mileage reimbursed at the government approved rate
• Average of 4.5 patient visits per day with approximately 20-25 patients
• Responsibilities include OASIS admissions, all time points, episode management, physician calls and coordination of care, timely EMR documentation, and clinical competencies in wound care, IV, ports, lab draws, and chronic disease management
• Charting experience with OASIS required; experience with Epic Remote Client/Remote Care preferred
• Minimum 2+ years of home health experience required
• Prior travel experience required
• OASIS certification is a plus
• Required certifications: AHA or ARC BLS
• Active Wisconsin or Compact nursing license required
• Valid driver's license required
• Associate Degree or Certificate required; Bachelor's Degree preferred
• Scrub color: Navy blue
