Opportunity:
Physical Therapist – Physical Therapy
Job Details:
• 8 variable day shifts, Monday through Friday
• Guaranteed 38 hours per week with flexibility up to 40 hours per week
• No holiday rotation required
• Physical therapists rotate one Saturday every 6 to 8 weeks with a compensatory day off during that week
• Occasional travel to central and northern regions required
• Mileage reimbursed at the government approved rate
• May be placed on low census on-call; call back minimum is 2 hours
• Evaluate, treat, and plan patient care in accordance with Physical Therapy standards of practice
• Serve adult and geriatric patient population with a variety of orthopedic, neurological, and functional mobility diagnoses
• Meet productivity expectations of an average of 4.5 visits per day
• Document to meet regulatory and reimbursement requirements using NetSmart EMR
• Use provided laptop for point of service documentation and cell phone for GPS and communication
• Scrub colors: any or business casual attire acceptable
• Required certifications: AHA or ARC BLS, valid Wisconsin state licensure, valid driver's license with proof of car insurance, and NPI number required
• Bachelor's degree in Physical Therapy required; minimum of a Master's degree preferred
• Minimum experience required in home health with training in adult and geriatric populations
• Knowledge of Physical Therapy standards and practices required
• Must live outside a 50-mile radius of the primary location
