Opportunity:
RN – Home Health Nurse
Job Details:
• 8-hour varied shifts with a guaranteed 37 hours per week with flex up to 40 hours per week
• Every 3rd weekend required
• Holiday rotation required
• On-call 1-2 times during the week and every other weekend, primarily triaging calls with a 15-minute response time requirement
• Travel required with an average of 4.5 visits per day across approximately 50 miles per day depending on territory, patient census, and staff availability
• Caseload of 20-25 patients
• Potential to float to other territories to assist when needed during low census
• Conduct OASIS admissions, all time points, and episode management
• Coordinate care including physician calls, documentation in EMR, and clinical competencies in wound care, IV, ports, lab draws, and chronic disease management
• Minimum 2 years of home health experience with travel experience required
• Associate Degree or Certificate required
• Required certifications: Wisconsin RN License, BLS (AHA or ARC), Valid Driver's License
• OASIS certification is a plus; experience with Epic Remote Client/Remote Care preferred
• Contractors must live outside a 50-mile radius
