Opportunity:
RN – Home Health
Job Details:
• Work 8-hour day shifts, typically 8:00 AM to 4:30 PM with possible flexibility based on census
• Guaranteed 40 hours per week with possible weekend and holiday call responsibilities
• Weekday on-call duties with potential for low census call-in
• Call back minimum of 2 hours when called back to work
• Travel to 20-25 patients weekly with 4-5 visits per day on average
• Daily travel can range from 25-60 miles, with potential for over 100 miles in large territory
• Mileage reimbursement at current government rate
• Required certifications: AHA BLS, active Michigan and Wisconsin RN licenses, valid driver's license
• OASIS certification preferred
• Minimum 2+ years home health experience preferred with travel experience required
• Charting experience in OASIS and Epic required
• Perform OASIS admissions, manage all time points and episodes
• Handle doctor calls and coordinate patient care
• Complete timely EMR documentation
• Maintain clinical competencies in wound care, IV therapy, ports, lab draws, and chronic disease management
• Contract eligible for 36 hours of unbillable low census hours
• During flu season, unvaccinated travelers must wear masks at all times per facility policy
