Opportunity:
RN – Case Management
Job Details:
• Work 8-hour day shifts from 8:00 AM to 4:30 PM with potential flexibility in start and end times
• 40 guaranteed hours per week over 13-week contract
• Every other weekend schedule required
• Holiday rotation required
• On-call duties 1-2 times during the week and every other weekend
• 15-minute response time required for answering service calls
• Travel 50-100 miles per day depending on territory and patient census
• Average 4.5 visits per day with 20-25 patients
• May float to other territories during low census periods
• Minimum 2 years of home health experience required
• Travel experience required
• Required certifications: Wisconsin nursing license, AH ABLS, valid driver's license
• OASIS certification preferred
• EPIC and OASIS charting experience required
• Perform OASIS admissions, episode management, doctor calls, and care coordination
• Clinical competencies required in wound care, IV therapy, ports, lab draws, and chronic disease management
• Must be dependable, accountable, and able to manage change effectively
• Flu and COVID vaccination required or must wear mask at all times per facility policy
• Contract eligible for 36 hours of unbillable low census hours
• Call back minimum is 2 hours when called back from on-call status
