Opportunity:
PT – Physical Therapist
Job Details:
• 13-week contract with 40 guaranteed hours per week
• Monday through Friday schedule from 8:00 AM to 5:30 PM
• Occasional travel required to central/northern regions with mileage reimbursed at government approved rate
• Rotate working one Saturday every 6-8 weeks (receive weekday off in exchange)
• No holiday requirements
• Subject to low census and on-call placement with 2-hour minimum callback
• Contract eligible for 36 hours of unbillable low census hours
• Master's Degree in Physical Therapy required
• Home health experience required with adult/geriatric population
• Experience with orthopedic, neurological, and functional mobility diagnoses
• Wisconsin state licensure and NPI number required
• Required certifications: AHA BLS
• Use laptop for point-of-service documentation and NetSmart EMR system
• Cell phone provided for GPS and communication
• Evaluate, treat, and plan patient care according to Physical Therapy standards
• Meet productivity expectations averaging 4.5 visits per day
• Document to meet regulatory and reimbursement requirements
• Business casual dress code or scrubs of any color
• Mask required during flu season if declining flu and COVID vaccination
