Opportunity:
RN – Home Health
Job Details:
• Work 8-hour day shifts, roughly 8:00 AM to 4:30 PM with potential flexibility based on census
• 36-40 hours per week flexible schedule
• Weekend and holiday call rotation required
• Weekday call 1-2 nights as needed, weekend call rotation
• Travel to 20-25 patients weekly with 4-5 visits per day on average
• Daily travel can vary from 25-60 miles depending on territory and patient census
• Mileage can exceed 100 miles due to large territory coverage
• Mileage reimbursement provided at current government rate
• Must live outside 50 miles from the facility (radius rule)
• Required certifications: BLS (AHA or ARC), active MI and WI RN licenses, valid driver's license
• OASIS certification is a plus
• Minimum 2+ years home health experience required
• Travel experience required
• Charting experience in OASIS and Epic required
• Perform OASIS admissions, all time points, episode management, doctor calls and coordination of care
• Handle clinical competencies in wound care, IV therapy, ports, lab draws, and chronic disease management
• Complete documentation in EMR in a timely manner
• Associate degree or certificate required, bachelor's degree preferred
• During flu season, unvaccinated travelers must wear masks at all times
