Opportunity:
RN – Home Health
Job Details:
• Work 5-8 hour variable shifts Monday through Friday
• 38 guaranteed hours per week with flexibility up to 40 hours
• 13-week contract position
• Provide home health nursing care to 20-25 patients with average of 4.5 visits per day
• Travel 25-60 miles per day depending on territory, patient census, and staff availability
• Primary coverage area includes Stevens Point and Plover regions
• Float to other territories when patient census is low
• On-call requirements: 1-2 times during the week and every other weekend
• 15-minute response time requirement for answering service calls
• Weekend and holiday rotation every other weekend
• Perform OASIS admissions, episode management, and all time points
• Coordinate patient care including doctor calls and care coordination
• Provide clinical competencies in wound care, IV therapy, ports, and lab draws
• Manage chronic disease cases and ensure timely EMR documentation
• Must live outside 50-mile radius from facility
• Required certifications: BLS (AHA or ARC), active Wisconsin or Compact nursing license, valid driver's license
• OASIS certification required, Epic experience preferred
• Minimum 2+ years home health experience required, travel experience mandatory
• Bachelor's degree required
• Any color scrubs acceptable
• Mask required at all times during flu season if not vaccinated for flu and COVID
