Opportunity:
RN – Home Health
Job Details:
• Work 8-hour variable day shifts, approximately 8:00 AM to 4:30 PM with potential flexibility based on census
• Guaranteed 36 hours per week with flex to 40 hours per week
• Every 4th weekend rotation and holiday rotation required
• On-call responsibilities 1-2 times during the week and every 4th weekend (at least once per month)
• 15-minute response time requirement for answering service calls
• Travel to see 20-25 patients with average of 4.5 visits per day
• Travel requirements can vary from 60+ miles per day depending on territory and patient census
• May float to other territories when needed due to low patient census
• Minimum 2+ years home health experience required, travel experience preferred
• Required certifications: BLS (AHA or ARC), Active Wisconsin or Compact nursing license, Valid driver's license
• OASIS certification required, Epic charting experience preferred
• Responsibilities include OASIS admissions, all time points, episode management, doctor calls and care coordination
• Clinical competencies required in wound care, IV, ports, lab draws, and chronic disease management
• Must demonstrate strong time management, resilience, dependability, and compassionate patient care
• During flu season, unvaccinated travelers must wear masks at all times per facility policy
• Associate degree or certificate required
