Opportunity:
RN – Home Health
Job Details:
• Work 8-hour day shifts, approximately 8:00 AM to 4:30 PM with potential flexibility based on census
• Guaranteed 40 hours per week for 13-week contract
• Every 5th weekend and holiday rotation required
• On-call responsibilities 2-3 times per month
• Serve 20-25 patients with average of 4.5 visits per day
• Travel 25-60 miles per day depending on territory and patient census
• Potential to float to Medford, Antigo, and Wisconsin Rapids locations
• Required certifications: BLS (AHA or ARC), Active Wisconsin or Compact license, Valid Driver's license
• OASIS certification preferred
• Minimum 2+ years home health experience required
• Prior travel experience required
• Charting experience in OASIS required, Epic preferred
• Perform OASIS admissions, episode management, doctor calls, coordination of care
• Provide clinical competencies in wound care, IV therapy, ports, lab draws, and chronic disease management
• Complete timely documentation in EMR
• Navy blue scrubs required
• Associate degree or certificate required, Bachelor's degree preferred
• Must reside outside 50-mile radius from facility
