Opportunity:
Physical Therapist – Physical Therapy
Job Details:
• Work Monday through Friday, 8:00 AM to 5:00 PM with no weekend rotation, call, or holiday expectations
• Position is 100% remote within the designated coverage area following approximately 2 days of onsite orientation
• All meetings are held virtually with no physical office to report to
• Expected to complete 6 patient visits per day within the designated coverage area
• Coverage area: Central Metro E zip codes 60630, 60618, 60625, 60646, 60659, 60660, 60657, 60640, 60613, 60614 and surrounding areas
• Patient population consists of adult and geriatric general med/surg type patients with a current census of approximately 200 patients
• Assist patients with ADLs and adaptive equipment
• Effectively collaborate with other specialties including RN, OT, and Speech
• EMR system: EPIC
• Supplies and equipment will be delivered to traveler's home
• Candidate must provide their own reliable transportation to and from all patient visits
• Must have a valid driver's license and active auto insurance
• Dress code: Any solid color scrubs acceptable or business casual attire; no jeans or open-toe shoes
• Minimum 2 years of Physical Therapy home health experience with adult and geriatric patients required
• Active Illinois PT license required
• BLS (AHA) certification required
Facility:
This organization provides comprehensive home health services to patients across a designated metropolitan coverage area. The team supports a census of approximately 200 patients, focusing on adult and geriatric general med/surg type cases. The interdisciplinary team includes nursing supervisors, managers, and team leads, and utilizes EPIC as their electronic medical records system to ensure coordinated and high-quality patient care.
