Opportunity:
RN – Admissions/Transfers
Job Details:
• 12-hour shifts with variable days or nights available; start times at 7:00 AM, 11:00 AM, 1:00 PM, and 7:00 PM
• Guaranteed 36 hours per week
• Every other weekend and every other holiday rotation required
• No float requirements
• Can be placed on low census/on-call; call back minimum is 2 hours
• Facilitate consults and transfers for patient placement across 16 hospital locations
• Coordinate patient admissions from other healthcare facilities, physician offices, and clinics
• Follow protocols, policies, and algorithms in accordance with EMTALA regulatory requirements
• Anticipate admission needs and communicate findings with transferring departments to optimize patient flow
• Coordinate across all disciplines for patient transfers including disposition, ambulance transportation, and specialist referrals
• Provide estimated time of arrival to both referring and accepting sites for timely handoffs between facilities
• Maintain accurate documentation and tracking of all patient transfers into and out of the system
• Collaborate closely with bed planning teams to efficiently accept critically ill patients
• Utilize critical thinking skills to ensure placement of the right patient in the right bed
• Remotely engage with facility staff to monitor, document, and communicate patient transfer needs
• Triage remotely using critical care or emergency department experience to identify patient care needs
• Utilize chain of command to advocate for priority admissions or transfers to higher levels of care
• Communication is a critical component of this role
• Team consists of 2 RNs working nights and 2-4 RNs working days
• Scrub colors: any color or business casual
• Minimum 3-5 years ICU or ER experience required; 5 years preferred
• Trauma Level II experience required
• Transfer call center experience highly preferred
• Associate degree or certificate required
• Required certifications: WI RN License, AHA or ARC BLS
• EPIC charting experience required

This facility is a major regional hospital and the flagship location of a large integrated health system serving communities across Wisconsin and Michigan. The facility offers a comprehensive range of acute care services and operates as a Trauma Level II center, supporting a wide network of affiliated hospitals. The Admissions and Transfer Center plays a critical role in coordinating patient flow and transfers across the entire health system, making it an important hub for patient care coordination.
