Opportunity:
RN – Home Health Nurse
Job Details:
• 13-week contract with 40 guaranteed hours per week
• 8-hour day shifts, approximately 8:00 AM to 4:30 PM with flexibility depending on census
• Every 5th weekend and holiday rotation required
• On-call required 2-3 times per month
• Average of 4.5 patient visits per day with 20-25 patients in caseload
• Travel of approximately 25-60 miles per day depending on territory, patient census, and staff availability
• Mileage reimbursed at the government approved rate
• Potential to float to Medford, Antigo, and Wisconsin Rapids on a limited basis
• Responsibilities include OASIS admissions, all time points, episode management, physician calls, coordination of care, and timely EMR documentation
• Clinical competencies required in wound care, IV, ports, lab draws, and chronic disease management
• Required certifications: AHA or ARC BLS, active WI or Compact license, valid driver's license
• OASIS certification is a plus
• Charting experience with OASIS required; experience with Epic Remote Client/Remote Care preferred
• Minimum 2+ years of home health experience required
• Prior travel experience required
• Associate Degree or Certificate required; Bachelor's Degree preferred
• Scrub color: Navy blue
