Opportunity:
Social Worker – Social Worker
Job Details:
• Work day shifts from 8:00 AM to 4:30 PM with occasional weekend requirements
• 40 guaranteed hours per week
• 13-week contract
• May be low censused and placed on call; call back minimum is 2 hours
• Work across Med Surg and Critical Care Clusters in all inpatient units using a Layered Model in tandem with RN Team for discharge planning
• Patient ratio of 8 to 20 patients per day based on acuity
• Minimum 1 year of experience in social services and counseling in a hospital acute care setting with a diverse population required
• Master's Degree in Social Work (MSW) preferred; Master's Degree required
• Charting experience in Epic required
• Wisconsin license required
• Holiday rotation as needed
• Business attire required
• Traveler must reside 50 miles outside of the facility

This hospital facility provides comprehensive inpatient care across multiple units including Medical Surgical and Critical Care Clusters. The organization utilizes a collaborative Layered Model approach where social workers work in tandem with RN Teams to support effective discharge planning for a diverse patient population. The facility uses Epic as its primary charting system and maintains high standards for patient care coordination.
