Opportunity:
RN – Home Health Nurse
Job Details:
• 13-week contract with 40 guaranteed hours per week
• 8-hour day shifts, approximately 8:00 AM to 4:30 PM with potential flexibility depending on census
• Every 5th weekend and holiday rotation required
• On-call required 2-3 times per month
• Average of 20-25 patients with 4.5 visits per day
• Daily travel ranges from 25-60 miles depending on territory, patient census, and staff availability
• Mileage reimbursed at the government approved rate
• Potential to float to Medford, Antigo, and Wisconsin Rapids on a limited basis
• Minimum 2+ years of home health experience required
• Prior travel experience required
• Required certifications: AHA or ARC BLS, Active WI or Compact license, Valid Driver's license
• OASIS certification is a plus
• Charting experience with OASIS required; experience with Epic Remote Client/Remote Care preferred
• Responsibilities include OASIS admissions, all time points, episode management, physician calls and coordination of care, timely EMR documentation, and clinical competencies in wound care, IV, ports, lab draws, and chronic disease management
• Associate Degree or Certificate required; Bachelor's Degree preferred
• Scrub color: Navy blue
