Opportunity:
Physical Therapist – Physical Therapy
Job Details:
• Monday through Friday schedule from 8:00 AM to 5:30 PM
• Guaranteed 37 hours per week with flexibility up to 40 hours
• 13-week contract
• Physical Therapists rotate one Saturday every 6-8 weeks with a corresponding weekday off
• No holiday rotation required
• No on-call or call-back responsibilities
• Occasional travel to other branches as needed with mileage reimbursed at the government approved rate
• Evaluate, treat, and plan patient care in accordance with Physical Therapy standards of practice
• Serve adult and geriatric patient population with a variety of orthopedic, neurological, and functional mobility diagnoses
• Meet productivity expectations of an average of 4.5 visits per day
• Document using a laptop for point of service documentation, NetSmart EMR, and cell phone for GPS and communication
• Required certifications: AHA BLS and State of Wisconsin licensure
• NPI number required; candidates without one must begin the application process prior to start
• Bachelor's degree required; Master's degree in Physical Therapy preferred
• Minimum experience in home health required with training working with adult and geriatric populations
• Knowledge of Physical Therapy standards and practices required
• Contractors must live outside a 50-mile radius of the primary location
