Opportunity:
RN – Home Health Nurse
Job Details:
• Work 8-hour day shifts from 8:00 AM to 4:30 PM with potential to flex start and end times as needed
• 40 guaranteed hours per week with mileage reimbursement
• 13-week contract
• Every 5th weekend required
• Limited on-call responsibility approximately once every other week, with a 15-minute callback response time requirement
• On-call duties primarily involve triaging calls by phone, with the clinician determining if issues can be resolved remotely or require an in-person visit
• Average of 20-25 patients with approximately 4.5 visits per day
• Travel between 50-100 miles per day depending on territory, patient census, and staff availability
• Potential to float to other territories to assist during low census periods
• Responsibilities include OASIS admissions, all time points, episode management, physician calls, coordination of care, timely EMR documentation, and clinical competencies in wound care, IV, ports, lab draws, and chronic disease management
• Charting in EPIC and OASIS experience strongly preferred
• Minimum 2 years of home health experience strongly preferred
• Travel experience preferred
• Bachelor's Degree required
• Required certifications: WI or compact RN license, AHA or ARC BLS, and valid driver's license
• OASIS certification is a plus; willing to train
• Holiday rotation required as needed
• 36 hours of low census unbillable hours available per contract
