Opportunity:
Physical Therapist – Physical Therapy
Job Details:
• Monday through Friday schedule from 8:00 AM to 5:30 PM
• 40 guaranteed hours per week
• 13-week contract
• Primary location is Medford with occasional travel to central and northern regions
• Mileage reimbursed at government approved rate
• May be placed on call during low census periods with a minimum 2-hour call back
• Physical Therapists rotate one Saturday every 6-8 weeks with a weekday off in exchange; no holiday rotation required
• Evaluate, treat, and plan patient care in accordance with Physical Therapy Standards of Practice
• Serve adult and geriatric patient population with a variety of orthopedic, neurological, and functional mobility diagnoses
• Meet productivity expectations of an average of 4.5 visits per day
• Document to meet regulatory and reimbursement requirements using NetSmart EMR
• Use a laptop for point of service documentation and a cell phone for GPS and communication
• Minimum of 1 year of home health experience required with training in adult and geriatric populations
• Master's Degree in Physical Therapy required
• Required certifications: AHA or ARC BLS, Wisconsin state licensure, and NPI number required
• Scrub colors: any color or business casual attire accepted
• Must reside outside a 50-mile radius from the facility
